Acceptance form - Service Partner (Makeup Artist)

Terms and conditions:

This partnership contract is for Makeup Artist valid for one year. The contract shall be renewed every year.
Any local booking assigned must fall under 12 km of radius from the Makeup Artist registered residence
Outstation bookings will only be assigned if the Makeup Artists opted for it.
No enrolment fees or hidden charges shall be applied upon or after on-boarding Makeup Artist as a service partner.
A commission of 0-10% per service will be levied towards the end of the monthly settlement based on the total earnings. No commission charged on total earnings upto Rs 10,000 per month.
Makeup Artist is expected to carry his/her own vanity of nail products.
Company does not pay for travel allowance.
Makeup Artist is free to withdraw anytime with a prior intimation.
On refusal of a booking assignment by the Makeup-Artist a penalty of Rs 30 will be charged on the next booking assigned. Bookings will be assigned only on days and time slots opted by the makeup artist.
Makeup Artist should not disclose un-necessary personal information to the client like Phone no, addresses etc and is responsible for his/her own security.
Contract will immediately end for the Makeup Artist on 3 successive refusals to booking assignments with no further discussion.
Contract will immediately end for the Makeup Artist for any misbehaviour displayed at the client site.
